Rich, spoilt New York nightclub owner J.P. Monroe becomes transfixed by a six-foot tall, intricately carved pillar on display at an art gallery. Deciding that he must have this bizarre artifact at his nightclub, J.P. quickly purchases the pillar from the shopkeeper.

Joanne "Joey" Summerskill, a young and ambitious television reporter, is just wrapping up another non-news story in a deserted hopsital emergency room when a semi-conscious teenage boy is rushed in. Dangling from his face and body are long, bloody hooks and chains. Before they can make a move to help him the boy is torn apart by the chains as if they were tugged by invisible hands.

Joey's only clue to the incredible death that she has witnessed is the boy's trendy girlfriend, Terri, who quickly disappears from the hospital. Joey quickly tracks the girl to a nightclub called The Boiler Room, and Terri reveals that the chain emerged from a small puzzle box.

When Joey and Terri investigate the origins of the puzzle box, J.P. accidentally spills a few drops of blood on the pillar, releasing the imprisoned soul of PINHEAD.
